67145 Retinal Tears

Is it ever appropriate to bill 67145 more than once during the 90 day post op period? What if the patient has a new tear?

Is the tear in the same eye? If the patient is returned to OR in a global period for a complication of initial surgery modifier 78 is attached to surgical code. If a different eye and unrelated to initial surgery (global) for different eye (ex, RT), then bill the procedure with modifiers 79 and (ex, LT).

If a procedure code states (one or more sessions), then subsequent treatment (ex, laser), is included in global for same eye.

The new tear is in the same eye. 67145 is a "one or more sessions" code.
