Depo provera--how to find out what to bill for a med

In order to answer this question, you need an up to date HCPCS book. It includes an alphabetic index of medicines. If the medicine you are asking about isn't there, look on line for the generic name of the service.

In this case, Depo-Provera has two codes: J10551 and J1055. Go to the listing and you'll see:

J1051 is: Injection medroxyprogesterone acetate 50 mg
Amen, Cycrin, Depo-Provera, Provera

and

J1055 is Injection Injection medroxyprogesterone acetate for contraceptive use 150 mg
Amen, Cycrin, Depo-Provera, Provera

Select the DOSAGE that you provided.

Add in the fee for the administration: 96372

This is no longer correct. The code changed for family planning after Jan 1 and J1055 is no longer recognized.
