Billing 99211 for an immunization

An insurance company is insisting that we bill a vaccine administration code for immunization visits. We have been billing 99211 for single shots. Conceptually these visits meet the definition for a 99211. Is this defensible?

Typically you would bill the code that best describes the service, so it's reasonable for the insurer to want the vaccine administration code in these instances. Some will allow 99211, but I'm not sure you have the basis to appeal from a policy perspective if all that is happening is the vaccination.

Absolutely agree that it is indefensible to bill for a nurse visit when service provided was an immunization administration.
