I think modifier 25 is good to go

Depends on what the visit is for. If it is because of a surgery, and depending on the global days, it could be not billable, if it is the decision for a surgery, probably modifier 57....
You just cannot put a "25" on it and let it go. It depends on the circumstances of the visit.
