For this combinations of services, CPT® codes 96372 is designated with indicator '1' in the NCCI edit list for E/M code. If this is a separate, distinct service (e.g., performed during a separate patient encounter), submit CPT® modifier 59 to indicate that the injection procedure is a separate service. Documentation in the patient’s medical record must support the use of this modifier. If the visit is related to 96372, do not bill E/M. Code 99211 can not be billed with 96372.

For Medicare we also need to bill drug code for which 96372 is billed. If patient brought drug from out of pocket then you need to bill drug code with $0.00 and also add KX modifier with 96372. If you are billing with OV then use 25 modifier with OV code.
