Billing for residents
When billing E/M's and the provider of service is a resident and the attending is supervising, how do we report and document the encounter? Thanks in advance for any insight.

If not the primary care exemption (GE), then the attending Must document physical presence with the patient for the history, or physical portion and must agree or disagree with resident findings (modifier GC). Example: I performed a history and physical examination of the patient and discussed his management with the resident. I reviewed the resident's note and agree with the documented findings and plan of care.
