Incision and Drainage with packing
I Incised and Drained a wound and ended up packing it. I realize there is a global period on this. Does anyone know what the global is on an I&D with packing..Many times we have the patient come back over numerous days to change the packing, but it goes on for a few weeks.. I thought the global was 10 days. If so, after the 10 days if I am still packing the wound (due to the depth and healing) can I start to charge and office visit for this? 99211 for nursing or 99212 If I go in and evaluate the patient, and or I change the packing myself? Thanks

You're right: all of the I&D codes listed below have a ten day global period: 10060, 10061, 10080, 10081, 10120, 10121, 10140, 10160, 10180, 11770.
Changing the dressing/re-packing is included in the payment for the procedure for ten days.
If the patient needs to be seen after the global period is over, you may bill a nurse visit (if a Medicare patient, that visit must meet incident to guidelines, so you have to be in the office when the nurse performs the service) or an office visit (99212-99215) based on the level of history, exam and MDM you perform.
