UDS(Urine Drug Screen)
My doctor's office performs UDS using 12 panel cup and I was told each strip is dipped in urine. This also a clia waived office. What would be the correct CPT® code and how many units would I bill for test as described to me. Also, they also use single strip(s) for Medicare patients, so what would be the correct code for Medicare?

What brand do you use? I am looking for one for the office. The manufacturer should have billing info- I think it is 80101 x number of tests

80101 x units is what I was thinking also. I was told 80104 but I don't it best describes the techique the way the office is testing. They dip the strips.

I bill Medicare G0434QW.

I bill Medicare G4034 QW modifier and for commercial carriers I bill the 80104 with the number of units normally my dr. bills 11 units and 98% of the time we are reimbursed except with BCBS

How are you testing UDS in your office and and do you have the cup/11 panels? I am still trying to understand. Thank you.

What code are you using for BCBS? I'm having major issues with G0434-QW being paid. I'm new to this practice and billing completely! Help! Thanks..

Please do I use 80101 or 80104 when the office has the 12 panel cup (strips dipped in urine)? We are a Clia Waived office. I use G0434QW for Medicare. Also, looked at the list of Clia Waived tests, I did not see 80104 with a mod "QW" so when I bill Medicaid, do I need to append modifier?
