The principal diagnosis is defined as the condition, after study, which occasioned the admission to the hospital (Source: ICD-9-CM Official Guidelines for Coding and Reporting, effective October 1, 2007, pp. 84–86)Jul 10, 2008

re: Please Help With Exam Questions

For a layered closure I would bill 12032, 12001 is for a simple repair and for size under 2.5 cm, since you would add together the sizes of the sites if using a simple closure you would use 12002.
