I&D follow up, please help

I know when a pt comes in for a follow up/re-packing you are not suppose to charge a office visit, under the 10day global. However what if more is actually done to the pt, eg: cleaned,more fluid expressed manually,anesthesia for re-packing, can you charge an office visit, since it wasn't just a "easy" dressing change? Thanks
