HCFA form help?

Good morning,

We recently brought on a nurse practitioner to our practice. Should her NPI or TID be entered anywhere on the HCFA 1500 form if she is seeing these patients in our office for the head physician?

If incident-to guidelines are met then the NP will bill the service under the physician's NPI.

If incident-to guidelines are not met then the NP will bill the service under his/her own NPI number.
