Billing CPT® 96401 to Missouri Medicaid

My provider billed two units of 96401 to Missouri Medicaid. the claim denied for incorrect units stating the allowable unit per day is one. per medicaid representative if the provider adminstered two units of chemo drug CPT® 96401 needs to be billed under two line item . Need clarification on the below;

1 if 96401 is billed under two line item do i need to append any modifier

2. Does Missouri medicaid policy wants 96401 needs to billed under two line item
