How to properly bill 58140 ,58925 and 58700 for inpatient

How to bill dx 620.2 ,218.9,625.9
58140
58925
58700
for inpatient .
I am not 100% sure if codes can all be billed together or which modifier I would need to use to ensure payment.
please reply soon.
Its very urgent . please respond anybody please ....

No one can tell you how to bill something without the Op report. You should research these codes by CPT® book and Google.

I agree with LBAROGIANIS... we need the operative report. FYI per NCCI edits 58700 is a column 2 code for 58140, therefore it CANNOT be billed separately at all. 58925 RVU's are less than 58140, so if documentation support the CPT® codes you have listed, append modifier 51 to 58925. Hope this helps!
