diaphragm in addition to insertion code 57170?

Can I bill the diaphragm in addition to insertion code 57170?

9CPT code 97605- Neg pressure wound therapy

We have a physician using this code in place of service 11 (and/or )and Highmark denies it for place of service. Does any one have this problem and if so how did you or are you rectifying it?
