history and physical office visits.
when an H & P is done at an initial office consultation for an urgent surgery, can we charge for that separate from the office consult. what CPT® would be used, if so.

per the CPT® guidelines: Whe the consultant assumes respnsibility for the management of any or all of the patient's care subsequent to the initial consultation, consult codes are no longer appropriate. So if your physician is taking over care then the approiate codes would be NP E&M codes depending on the location; eg, initial in-patient E&M( if its outpatient surgery then you would use the E&M office visits 99201-99205). Per the surgery guidelines in CPT® if the visit lead to the decision for surgery then you would append a modifier 57 to the E&M level, this includes the H&P.
