Will medicare pay for medicine (J codes 7611-7614) with nebulizer treatments in the office?
I am getting conflicting answers re if medicare part B will pay for medications dispensed with nebulizer treatment in the office or outpatient clinic setting. These J codes (7611-7614) fall under the medical coverage determinator D. As an example: If a physician billed a 94640 would you also bill J7611? If not, can someone identify where I would find something specifically in writing for my docs?

with the allergy season in full swing, this has come up again and again in my office.
i too am receiving conflicting answers. for medicare, is the med bundled into the cpt code for the treatments?

Bill the drug separately. If an E/M was also medically necessary use modifier -25 on E/M code.

Palmetto is denying saying we need to bill as DME. I am confused as to why, when it was our machine used in our office.

J7604 - J7699 Inhalation Solutions Local Carrier if incident to a physician's service. If other, DME MAC.
