Denial of V58.69
What diagnosis could possibly be primary for CPT® codes 99212, 85610, & 36416 if V58.69 is denied?

Did your doctor document the reason/condition for which the meds are prescribed? If so, I would put that primary to a V code

Another point, if the INR is being drawn can you use V58.61? What is the reason for the office visit? If it is separate from the blood draw, you should use that diagnosis and 99212 should not be denied.

V5861 is more specific to the anticoagulant use and will cover the pro-time
