•Time is the controlling factor (rather than history, examination, medical decision-making) when greater than 50% of the encounter is spent counseling or coordination care.
•In the office and other outpatient settings, the time is face–to-face with the patient.
•In the hospital or nursing facility, the time it the floor/unit time.
• Documentation needs to indicate:
- Total amount of time spent (face-to-face or unit/floor)
- The fact that greater than 50% was counseling or coordinating care.
- A description of the counseling or the activities to coordinate care.
