Maximum number of modifiers with one CPT® code

Hi All,
Hope everyone is doing great. Can anyone help me out regarding how many Modifiers we can bill with one CPT/HCPCS code for both paper and electronic claims? An early reply will be highly appreciated. Thanks sam

Hi there, just thought I would put 2 cents in here. I have put as many as 3 modifiers on a claim. I did only due to the parameters of the software I was using would not allow more. I have had to use a KX, GP and a 59 all for one claim for Medicare. I am sure if required by them you could go more. :))
