Cortical Mapping during Craniotomy

Can someone please tell me what specifications need to be met for a neurosurgeon to bill 95961 in the OR during surgery. Do they have to dictate some sort of report in addition to the OP note or can they just dictate the total time they monitored the EEG in the OP note's body.

Operating surgeon cannot bill for 95961

To bill separately for intraoperative neurophysiological testing, Medicare requires that this test be requested by the operating surgeon, and the monitoring of the patient's evoked responses must be performed by a physician other than the:

Operating surgeon;
Technical/surgical assistant; or
The anesthesiologist rendering the anesthesia.
