E&m w/cerumen removal & audio

Can we bill cerumen removal (69210) with audio and E&M if we have different diagnosis?

Ceremun removal

I have used an E&M code with a modifier 25 and a different diagnosis and have been paid for removal of wax 69210.

Cerumen Removal

It is my understanding that 69210 may only be billed when the cerumen requires removal by a Physician/NPP. Lavage of the ears by a Nurse is not reimbursable using this coe.
