How many times can you use modifier 76 when billing 90460 and 90461

Without looking it up, I don't believe there is a limit. With the exception of BS of CA, we have never had any problems using it.

Here's an example of a 4 mo. WB claim:

BS of CA prefers this:

99391-25
90723
90670
90648
90681
90460 x4
90461 x5
