E/M Code Bundled with Radiology Code

I've received a denial from a commercial carrier due to bundling. The E/M code billed was 99214 along with 73721-26. I'm not understanding how this could be bundled. Any suggestions?

adding a -25 to the E&M?

E/M Code Bundled with Radiology Code

You shouldn't have to add a 25 modifier to an E/M for x-rays. Are you sure the reason it was denied was because of these two codes bundling?
