CPT 94664

I read your article on Nebuliser education CPT 94664 and was wondering if an order would be required if performed hospital outpatient. ? Scenario.. patient comes in for PFT's and during the testing the therapists notice that the patient is not utlising the the Nebuliser correctly..... they ask the Rehab therapist / RN to come and give education to the patient. Should a physician first put in an order or is it ok if he documents in a note that he referred the patient to the rehab therapist for education? Or can the therapists add to to her education note that the patient was referred by DR. XX for education ? Our MAC is NGS and I could not find any guidance. Thanks Kars

There must be a specific written order by the physician for all respiratory care (respiratory therapy) services documenting the medical necessity of the education. To verify this check out your local carriers LCD or commercial payer guidelines. You also need to consider which services or components are covered and considered medically necessary as a part of a pulmonary rehabilitation program.
