Hello Everyone!

We are a hemophilia treatment center that dispenses clotting factor to patients to be self infused for either prophylaxis or as needed for bleeds. We do not have a licensed pharmacy, but reside in WI, which is classified as a physician dispensing state, therefore are not required to obtain a license for pharmacy. We do not mix any drug, strictly buy and bill for vials dispensed to patients. Therefore, in most cases, we bill to through the medical policy.

I have been using the POS 12, which I know is not accurate for this service. My instinct is to use POS 1, but my pharmacist has some hesitations that this would require that we possess a pharmacy license. The way I read CPT® is that that is not required unless specified by the payer.

Can I please get some insight from some of you more experienced coders out there? I'd really appreciate your take on it.
